Blister Prevention: What the Research Actually Says (vs What Hiking Forums Tell You)

The Problem with Most Blister Advice
Most hiking forums operate on tradition, not data. Someone uncle wore two pairs of cotton socks for 30 years, so that becomes the recommended strategy. But when researchers actually test these methods, the results often contradict popular wisdom.
A 2014 randomized controlled trial published in the Journal of the Royal Army Medical Corps assigned 357 army recruits to wear either cotton or acrylic socks during basic training. The acrylic group experienced a 48 percent relative reduction in new blisters (25 percent versus 48 percent). In civilian settings, marathon runners randomized to acrylic-polyester blends reported 32 percent fewer plantar blisters compared with cotton controls. The message is clear: cotton kills. Not dramatically, but consistently.
Sock Layering: The Single Most Effective Change
The strongest evidence supports a two-layer sock system. A thin synthetic liner against your skin, with a merino wool or merino-blend outer sock over it. The liner shifts the friction interface from skin-on-sock to sock-on-sock, which is far less damaging to your skin.
One study found that a dense double-sock system reduced overall blister incidence to 40 percent compared with 69 percent in the standard single-sock group. It also halved severe blister cases. Hydrocolloid patches (the clear puffy bandages) have stronger evidence: 40 to 57 percent reduction in blister formation when applied to high-risk areas before activity. They stay on for hours, even in wet conditions, and create a protective barrier that absorbs shear.
The Complete Evidence-Based Protocol
Based on the research I reviewed, here is the protocol I now follow before every hike of 10 kilometers or more:
- Socks: Injinji toe liner plus merino-blend outer sock. No cotton at any layer.
- Powder: Antifungal or foot powder applied before putting on socks.
- Hot spots: Hydrocolloid patch applied to any area that has blistered before, applied before leaving the house.
- Stop at the first sign: If I feel a warm spot, I stop immediately and apply a patch or sports tape. Not after 10 more minutes of walking. Immediately.
- Air breaks: At rest stops, I remove my shoes and socks for five minutes to let my feet dry.
